• You are assigned a government funded Home Care Package or transfer your Home Care Package from another provider, and enter into an Agreement with VMCH to become your Home Care Package (HCP) provider (HCP Agreement) on or before 01 November 2025.  
• You will receive a $250 Coles Myer gift card for personal use at two milestones, provided that VMCH has remained your Home Care Package provider from the date you signed your HCP Agreement: at three months after signing your HCP Agreement: and at six months after signing your HCP Agreement.
• Mention this offer at the time of signing your HCP Agreement.
