Active Minds exists to help people with dementia, who experience mild to moderate cognitive impairments. This might mean having difficulty with cognitive tasks like planning, thinking or remembering.
To be eligible to participate in Active Minds you will need to belong to the Commonwealth Home Support Programme (CHSP) and have the relevant Occupational Therapy referral code.
Talk to our friendly team to find out if Active Minds suits your needs.
Active Minds sessions are facilitated by an Occupational Therapist with activities tailored to participants’ abilities. The 75-minute sessions involve music, memories and games to help boost thinking and attention skills.
Our Active Minds Program goes into detail about how we help you look after your brain.
